Malpractice insurance is less than 1% of all healthcare costs. MRIs , and other expensive procedures, are sometimes ordered for purely "CYA" reasons. Whether or not these procedures are warranted is unknown. No studies about their effectiveness has ever been done.
You're right about pharmaceutical companies pushing "off label" uses and, quite frankly, in pursuit of the almighty dollar, tnese companies do push drugs for conditions for which the drugs have never been tested.

We have had the option of private insurance for many years. It is just not affordable for many people.
When I think of small businesses, I think of companies with less than 20 people. Few of those companies are able to offer health insurance without bankrupting themselves.
