Depends on what you buy. We have our system from Comcast. The DVR box replaces your existing cable box and costs $9.95 per month. The upside to that is, if something happens to the DVR they replace it free because basically you "rent" the box from them. In a year, we've gone through two boxes, so IMO its the way to go. S

I have my box through directv. My box is free I think- But I do pay 5.00 a month or something like that for the boxes upstairs (I did get 2 dvr boxes free when I signed up) I LOVE it- dh and I say all the time that we do not know how we lived before we could pause tv. For example- we love the show heroes, but couldn't watch it on Monday. We taped it, and were able to leasurely (sp?) watch it last night, and fast forward through all the commercials. We went through 2 boxes as well when we had dvr through charter, and have had a few problems with our downstairs box (freezing etc). I would reccomend having some kind of insurance because I have heard that they break easy. Charter compared them to a computer.
