I work in billing for a hospital. Most of the insurance companies we call for claim status, etc, are not located in the US. The lady I was on the phone with today said it was 4am there. It was 3pm CST here. That was a Medicare HMO insurance. Any time we call on a Blue Cross, it is directed to another country. The supervisors are here in the US, the claims are sent to US addresses, but all the call centers are in other countries. They have all the policy holder's info right there because they have us verify name, date of birth, social security #, address, and policy #. We can barely understand them half the time. Very few of the big companies have call centers in the US anymore.
