I don't have Pay Pay will mail you 2 of these forms for 2 stamps (one for the forms and one stamp to mail to you). Have only enough of these for 6 deals. Please mention mycoupons when emailing me.

I called the 800 number found there were only 2 stores in MN that had these forms.
Rainbow foods and Pamida. Neither one of these stores is where I shop.
She told me that the CRT had to be from one of the two stores.
Anyone else used a CRT from a non retailer listed?
