I read the entire mailer and did not see a phone #, but I made copies (front & back) of the 'reply form' and 'reply envelope' before mailing them off. There are 2 different addresses listed, Perryville, MO (inside the mailer) and Norwalk, CT (on the reply envelope). I'll be sending a postcard to each to insure cancellation.
