It may be necessary to access the survey through Zoomerang. I completed the survey and there was no information about a gift card at the end.

It asked me for my e-mail address at the end. We'll see if I get the gift certificate.

survey for a Healthwise diabetes product.

Healthwise is a nonprofit organization with one mission—to help people
like you make better health decisions. We write and produce health
information and products for consumers. It’s important to us that our
products work well for people, so we value input like yours.

The survey you responded to was originally distributed among the
members of two online diabetes forums. The survey was intended to have
a limit of 100 respondents. Unfortunately due to a technical error,
the automatic quota limitation failed. As a result, the survey link
was forwarded to people beyond the two forums.

We received more than 3,300 responses to the survey. Although we
appreciate your interest in Healthwise, we are a small nonprofit
organization and are unable to offer the incentive to those who
responded beyond the intended quota.

We regret to inform you that people who received the survey from
another source or were not among the first 100 respondents will not
receive the incentive.

Healthwise will protect your confidentiality and any information you
provided will not be counted as data and will be permanently deleted.
