Dateline: Friday, 10/23/2009. + My prescription was filled at: Big K Bagley Road and Engle Road intersection Middleburg Heights, OH 44130 + Included in the printout material of my precious prescription … KMart $5.00 gift card for feedback. Complete a survey about your KMart Pharmacy purchase and receive via mail a $5.00 KMart gift card. This offer is limited to the first 200 respondents. Respondents must be 18 years or older. Only one per address, do more than one survey and you will not get subsequent gift cards. + Using a touchtone telephone dial: (800) 721-4124 Project code: xxxxx (edited by original poster, it turns out I was never asked this project code) + You input your telephone number and then are connected to a live operator (by a subcontracted company) that will ask you feedback questions (on a scale of 01 to 10) that will last for 10 to 12 minutes.
